The Role of CLA Nursing Services

Families and providers sometimes have questions about the role of CLA nursing services and how nurses support the people we serve. CLA nurses work alongside Host Home Providers, staff, and healthcare professionals to help ensure individuals receive safe, coordinated medical support.

Below is an overview of how CLA nurses support individuals, providers, and staff.

What CLA Nurses Do
CLA nurses play an important role in supporting the health and well-being of the people we serve. Their work focuses on coordination, guidance, training, and oversight, rather than providing routine daily medical care.

While Host Home Providers typically schedule medical appointments and manage day-to-day care, CLA nurses provide clinical guidance, training, and collaboration with medical professionals to help ensure care is safe and well-coordinated.

CLA nurses help ensure that individuals receive appropriate medical care by:
  • Serving as a clinical liaison between PRS, Host Home Providers, staff, and medical professionals when health questions or concerns arise.
  • Tracking and reviewing health updates, including changes in medications or health conditions.
  • Reviewing physician recommendations and helping ensure care plans and medical instructions are clearly understood and coordinated.
  • Providing training and education to Host Home Providers and staff on medication administration, medical protocols, and health-related supports.
  • Delegating certain medical tasks when appropriate, (for example g-tube feedings) and ensuring staff are trained and competent to perform them safely.
  • Serving as a resource for questions related to health, medications, and medical care.

What CLA Nurses Do Not Do
CLA nurses are not intended to replace the role of a primary care provider or manage routine medical communication.

In most situations:
  • Host Home Providers should contact the individual’s doctor directly for medical concerns, appointment scheduling, or follow-up questions.
  • Nurses do not provide routine daily medical care in the home.
  • Nurses do not replace emergency services or medical providers.

If additional support or coordination is needed, the nurse can help guide next steps.

Working Together
The goal of CLA nursing services is to support Host Home Providers and staff with guidance, training, and coordination, while ensuring individuals receive appropriate care from their medical providers.

When providers, families, staff, and medical professionals work together, we can support the best possible health outcomes while helping each person continue living safely, independently, and as part of their community.
